Recurrent Implantation Failure - Causes and Treatment
Recurrent Implantation Failure: Why Does an Embryo Transfer Fail?
When an IVF cycle does not result in pregnancy, couples often wonder why the embryo did not implant. If this happens after repeated embryo transfers, a fertility specialist may recommend reviewing the previous treatment cycles to understand whether any underlying factors need attention.

What Is Recurrent Implantation Failure?
Recurrent implantation failure refers to repeated unsuccessful embryo transfers during IVF. There is no single definition that applies to every patient. The number of transfers, embryo quality and individual medical history all help determine whether further investigation is needed.
One failed transfer does not necessarily mean there is an ongoing implantation problem.
Why Might an Embryo Not Implant?
Several factors can contribute to unsuccessful embryo transfer:
-
Uterine conditions: Fibroids, polyps or scar tissue inside the uterus may interfere with implantation.
-
Embryo chromosome abnormalities: An embryo can look healthy under a microscope but still have chromosomal abnormalities.
-
Uterine lining: The condition of the endometrium and the timing of embryo transfer may influence the outcome.
-
Other medical factors: Certain underlying health conditions may affect fertility and pregnancy.
In some cases, no specific cause is found, even after a detailed assessment.
What Tests May Be Needed?
Your fertility specialist may review previous IVF reports and recommend investigations based on your medical history. These may include:
-
Ultrasound examination of the uterus and reproductive organs
-
Hysteroscopy if a closer examination of the uterine cavity is needed
-
Review of embryo grading and previous transfer records
-
Selected blood tests when indicated
-
Discussion of embryo genetic testing in appropriate cases
Not everyone needs all these tests. The choice depends on the findings and whether a particular investigation is likely to help guide treatment.
Can Embryo Genetic Testing Help?
Some embryos have chromosomal abnormalities that cannot be identified by their appearance alone. Preimplantation genetic testing for aneuploidy (PGT-A) may be considered in selected cases.
However, PGT-A is not suitable for every patient and does not guarantee pregnancy. Your specialist can explain whether it may be useful based on your age, treatment history and available embryos.
What Are the Next Steps After Failed Transfers?
The next step is to review what happened during previous IVF cycles and decide whether any changes are appropriate.
Depending on the findings, your fertility specialist may recommend treating a uterine condition, reviewing the timing of embryo transfer or discussing other fertility options. Sometimes, another transfer may be reasonable without additional treatment.
Avoid starting immune treatments, blood thinners or other add-on procedures without a clear medical indication.
